From Bourne Bridge:

  • After the bridge, take Rt 28/MacArthur Blvd S
  • Take right on Paesano Place and an immediate left into  Community Health Center at 123 Waterhouse Rd

 

From Falmouth & Mashpee:

  • From Route 28 at the Otis Rotary, take Rt 28/MacArthur Blvd North
  • Cross over Route 28 S at Trading Post Corner (Atlantic Subaru) to Waterhouse Rd
  • Community Health Center is at 123 Waterhouse Rd (Paesano Place) on the right

Transportation assistance is also available to and from all Community Health Center sites at no cost to our patients. Please call 508-477-7090 if you need transportation assistance.
